How GLP-1 weight loss medications work

GLP-1 medications copy a hormone your gut releases after you eat. They signal fullness to your brain and slow how quickly food leaves your stomach, so most people feel satisfied with less food and think about food less often. Many describe the quieter “food noise” as the biggest surprise.

Most are a once-weekly injection you give yourself at home with a short, thin needle. Two are now daily pills. Doses start low and step up every four weeks or so, which keeps the most common side effects, mainly nausea and other stomach upset, more manageable.

Which medication is which

Several of these are the same molecule sold under different names for different conditions.

Brand-name or compounded?

Brand-name GLP-1s such as Wegovy and Zepbound are FDA-approved and made by their manufacturers. Compounded versions are prepared by state-licensed pharmacies for an individual prescription. They usually cost less, but they are not FDA-approved and the FDA does not review them for safety, effectiveness or quality. Our compounded GLP-1 guide covers what changed in 2025 and how to choose a program carefully.

What to look for in a provider

  • The price at your maintenance dose, not just month one. Programs that charge the same at every dose are easier to budget for.
  • A real clinical review. A licensed clinician should review your history and adjust your dose when side effects hit or progress stalls.
  • A licensed US pharmacy, and a clear statement of whether your medication is compounded or FDA-approved.
  • Terms you can live with. Check for prepaid terms, cancellation windows and what happens if you are not prescribed.

Not sure which program fits?

Answer five quick questions about budget, medication and insurance, and we’ll rank the programs for you. No email or sign-up needed.

Questions, answered

What is the best online GLP-1 program?

It depends on what matters most to you. If you want the lowest predictable price, look for a program that charges the same at every dose. If you want FDA-approved brand-name medication, choose a program that prescribes Wegovy or Zepbound and helps with insurance. Our ranked list above shows how each program scores on price, care, medication options, transparency and experience.

Are online GLP-1 prescriptions legitimate?

Yes, when a licensed clinician in your state reviews your health history before prescribing. Look for programs that are LegitScript certified, use licensed US pharmacies, and tell you clearly whether a medication is compounded or FDA-approved.

How much do GLP-1 programs cost per month?

Taking each program’s lowest ongoing monthly price with medication and any required membership, compounded semaglutide runs $65 to $299 a month and compounded tirzepatide $85.58 to $378.08. Brand-name options usually cost more without insurance: manufacturer self-pay prices run from $149 a month (Wegovy pill or Foundayo at starting doses) to $449 (Zepbound at higher doses, with on-time refills). Always check whether the price changes as your dose increases.

Do I need insurance to get a GLP-1 online?

No. Most online programs are cash-pay and many accept HSA or FSA cards. Some programs help you use insurance for brand-name medication through prior authorization, which can lower the cost if your plan covers weight loss medication.

How do online GLP-1 programs work?

You fill in a health questionnaire, and a licensed clinician in your state reviews it, sometimes with a video visit or lab work. If a GLP-1 suits you, the prescription goes to a pharmacy that ships the medication to you, and the program follows up as your dose rises. Programs differ in how often a clinician checks in and whether labs are included.

What should I check before I join a GLP-1 program?

Five things: the price at your maintenance dose, not just the first month; whether a membership is billed on top; whether the lowest price needs a prepaid plan; the refund and cancellation terms; and whether the medication is compounded or FDA-approved. 12 of the 25 programs we rank keep one price at every dose.

Who qualifies for a GLP-1 prescription for weight loss?

Wegovy and Zepbound are approved for adults with a BMI of 30 or more, or 27 or more with at least one weight-related condition such as high blood pressure or type 2 diabetes, alongside diet and exercise. A clinician also checks for reasons you shouldn’t take one. See the Wegovy label and the Zepbound label.

What are the most common side effects of GLP-1 medications?

Nausea, diarrhea, vomiting, constipation and stomach pain, most often while the dose is rising. The approved semaglutide and tirzepatide labels carry a boxed warning for thyroid C-cell tumors, and people with a personal or family history of medullary thyroid cancer or MEN 2 shouldn’t take them. See GLP-1 side effects.
