In short

  1. As of October 27, 2026, Costco lists the Wegovy pen at $199 a month for the first 2 fills at 0.25 mg to 0.5 mg, then $349 at 1 mg to 2.4 mg.
  2. The Wegovy pill is $149 a month at 1.5 mg through December 31, 2026, then $199, and $299 at 9 mg to 25 mg.
  3. Ozempic is $199 for the first 2 fills, $349 at 1 mg and $499 at 2 mg; it’s approved for type 2 diabetes, so weight-loss use is off-label.
  4. Costco lists no member price for Zepbound; LillyDirect sells it for $299 to $449 a month with refills within 45 days.
  5. Costco’s member prices are cash only, can’t be combined with insurance, Medicare or Medicaid, and match NovoCare’s self-pay prices at each dose Costco lists.

As of October 27, 2026, Costco Pharmacy’s member prices put the Wegovy pen at $199 a month for the first 2 fills, then $349, the Wegovy pill at $149 or $299 depending on dose, and Ozempic at $199 to $499 a month. Those prices match Novo Nordisk’s own self-pay prices at NovoCare, and Costco lists no member price for Zepbound, which Lilly sells for cash through LillyDirect instead.

Below are Costco’s prices by drug and dose, who can use them, and how they compare with buying from the makers.

A month of the Wegovy pen is 1 box of 4 pens, and a month of the pill is 1 bottle of 30 tablets, per NovoCare. Prices change often, so check Costco’s price search for your prescription before you fill it.

Costco Wegovy price

For the Wegovy pen, the first 2 fills at the 0.25 mg and 0.5 mg starting doses cost $199 each, and every fill after that costs $349 through 2.4 mg. If you start before the offer ends on December 31, 2026, your first 12 monthly fills come to $3,888: 2 at $199 and 10 at $349.

The Wegovy pill starts at $149 for the 1.5 mg dose and costs $299 at 9 mg and 25 mg. Costco doesn’t list the 4 mg pill or the 7.2 mg Wegovy HD pen; NovoCare charges $199 and $399 for those. Our Wegovy cost guide has the full dose ladder.

Costco Ozempic price

Costco lists Ozempic at $199 for the first 2 fills at 0.25 mg to 0.5 mg, $349 at 1 mg and $499 at 2 mg, the same as NovoCare’s self-pay offer. Ozempic and Wegovy both contain semaglutide, but they’re approved for different things. The Ozempic label covers blood sugar control in adults with type 2 diabetes and lowering heart and kidney risks in that group. The Wegovy label covers weight reduction, lowering heart risk in adults with heart disease and overweight or obesity, and the liver disease MASH. Prescribing Ozempic for weight loss is off-label. Our Ozempic cost guide covers it in more detail.

Costco Zepbound

Costco’s member pricing doesn’t include Zepbound. Its Wegovy price page carries banners for Zepbound, Mounjaro and Foundayo that link to Lilly’s own savings pages instead. For cash buyers, Lilly sells the Zepbound vial only through LillyDirect self-pay, with home delivery or pickup at Walmart Pharmacy, and the KwikPen through LillyDirect self-pay with the same two options. LillyDirect lists four dispensing pharmacies: Amazon Pharmacy, Fuze Health, Gifthealth and Walmart Pharmacy.

LillyDirect’s self-pay prices, read October 27, 2026, are $299 at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg to 15 mg if you refill within 45 days. Refill later and 7.5 mg costs $499, and 10 mg to 15 mg cost $699. If your insurance covers Zepbound, Lilly’s savings card for the single-dose pen can bring it to as little as $25 a month through LillyDirect “and other pharmacy options” (Lilly); people with government insurance can’t use it. Our Zepbound cost guide has the year totals.

Who can get Costco’s member prices

Costco Member Value Pricing is “a direct-to-members savings program administered by Costco Pharmacy and Ventegra,” according to Costco’s pricing page. Three conditions follow from its terms:

  • You need a Costco membership. The prices are for members.
  • You pay cash. The program isn’t insurance, and claims sent to publicly funded programs such as Medicare and Medicaid, or to private insurance, aren’t eligible. You can’t stack the member price on top of a copay.
  • You need a prescription. Costco fills a prescription from your clinician. Its partner Sesame lists $29 virtual visits for Costco members for new prescriptions or refills, plus 10% off its other services, after you verify your membership (Sesame).

Paying cash has one more cost: NovoCare notes that money spent outside your insurance doesn’t count toward your deductible or out-of-pocket limit.

Sesame is one of the 26 programs we rank. Its weight-loss subscription is separate from the $29 visit: $99 a month, or $59 a month on the annual plan, with medication billed on top, as of November 2026.

Costco vs NovoCare and LillyDirect

For Wegovy and Ozempic, the price is the same whether you pick up at Costco or order from NovoCare, which offers home delivery or pickup at a pharmacy you choose. The differences are what each lists and when offers end:

  • Costco needs a membership; NovoCare doesn’t.
  • NovoCare lists every dose, including the 4 mg pill and Wegovy HD; Costco’s page doesn’t list those two.
  • Costco says its $149 price for the 1.5 mg pill runs through December 31, 2026 and becomes $199 after. NovoCare’s page lists $149 for that dose without an end date.
  • For Zepbound, LillyDirect is the cash route, since Costco lists no member price.

Options that may cost less

  • Commercial insurance with a savings card. If your plan covers Wegovy or Zepbound, the makers’ savings offers can bring the cost to as little as $25 a month, with NovoCare capping its savings at $100 a month. Government insurance is excluded. See our Wegovy coupon and Zepbound coupon guides.
  • Medicare. Eligible Part D enrollees pay $50 a month through the Medicare GLP-1 Bridge from July 1, 2026 to December 31, 2027, for Wegovy, Foundayo or the Zepbound KwikPen.
  • Compounded programs. At the 26 programs we rank, as of November 2026, compounded semaglutide costs $65 to $299 a month and compounded tirzepatide $85.58 to $378.08, at each program’s lowest ongoing all-in price. Compare them on our cheapest GLP-1 rankings.

A compounded GLP-1 is not Wegovy, Ozempic or Zepbound, and not a generic of them. A compounding pharmacy prepares it, it isn’t FDA-approved, and the FDA does not review compounded drugs for safety, effectiveness or quality (FDA).

The bottom line

Costco’s GLP-1 prices are the makers’ cash prices with a membership attached: the Wegovy pen at $199 for 2 starter fills, then $349, the Wegovy pill at $149 to $299, and Ozempic at $199 to $499, as of October 27, 2026. Costco lists no member price for Zepbound, which LillyDirect sells at $299 to $449 a month with refills within 45 days.

Costco makes sense if you’re a member and prefer warehouse pickup. If you have commercial coverage or Medicare, check that first, since a covered prescription can cost $25 or $50 a month. Talk with a licensed clinician about which drug and dose suit you, and see our guide to GLP-1s without insurance for every cash option.
