In short

  1. As of October 27, 2026, LillyDirect sells the Mounjaro single-dose pen for $499 a month self-pay at all six strengths, 2.5 mg to 15 mg. A year of 13 fills costs $6,487.
  2. Lilly lists Mounjaro at $1,112.16 per fill of 4 pens. That is the price Lilly charges wholesalers, not what most people pay.
  3. With commercial coverage, Lilly’s savings card can bring a 1, 2 or 3-month fill to $25, with savings capped at $150 a month and $1,950 a year, until December 31, 2026.
  4. Lilly says about 8 in 10 Medicare Part D patients pay $0 to $50 for a 28-day supply of Mounjaro, and the rest pay about $262 on average.
  5. Mounjaro is approved for type 2 diabetes, so using it for weight loss is off-label. Zepbound, the same molecule approved for weight loss, costs $299 to $449 a month self-pay if you refill within 45 days ($499 to $699 if later).

Mounjaro costs $499 a month if you pay cash at LillyDirect, Eli Lilly’s own pharmacy service, and the price is the same at every dose from 2.5 mg to 15 mg, as of October 27, 2026. With commercial insurance that covers it, Lilly’s savings card can bring a fill to $25; on Medicare Part D, Lilly says most people pay $0 to $50 a month. The list price is $1,112.16 per fill, but few people pay that.

This page covers Mounjaro the brand: what each dose costs, what changes with insurance, and why it isn’t the cheapest Lilly option for weight loss. For every form of the molecule side by side, including compounded versions, see our tirzepatide cost guide.

A month is one carton of 4 single-dose pens, or 28 days. On the Mounjaro label, adults start at 2.5 mg once a week and can step up by 2.5 mg after at least 4 weeks on a dose if blood sugar needs more control, to a maximum of 15 mg. Because the self-pay price is flat, moving up a dose doesn’t change what you pay.

The label also lists vials and a KwikPen, but LillyDirect lists its self-pay price for the single-dose pen, and Lilly’s savings card covers only the single-dose pen.

List price vs what you pay

Lilly puts Mounjaro’s list price at $1,112.16 per fill, where a fill is a month’s supply of 4 pens. Lilly says the list price, also called the wholesale acquisition cost, “is the price at which Lilly sells its products to wholesalers,” and that what patients pay depends mostly on their insurance, pharmacy charges and any assistance they use. Lilly took the figure from wholesale data accessed on January 1, 2026.

So the number you see on a pharmacy quote or an insurance statement may be close to $1,112.16, while a cash buyer at LillyDirect pays $499 and a covered patient with Lilly’s card may pay $25.

Mounjaro cost with insurance

Your insurance decides most of the price. In brief:

  • Commercial insurance that covers Mounjaro: Lilly’s card lets you pay as little as $25 for a 1, 2 or 3-month fill. It pays at most $150 per month of supply and $1,950 a year, for up to 13 fills. If your share is $400 a month, the card pays $150 and you pay $250. You need a prescription for an FDA-approved use, which for Mounjaro is type 2 diabetes.
  • Commercial insurance that doesn’t cover it: the card brings a 1-month fill to as low as $499.
  • Medicare Part D: Lilly says Mounjaro is covered on many Part D plans for type 2 diabetes, and that about 8 in 10 Part D patients pay $0 to $50 for a 28-day supply, with the rest averaging about $262 (IQVIA data, October 2024 to September 2025). The Medicare GLP-1 Bridge doesn’t list Mounjaro; it sends anyone with type 2 diabetes to their regular Part D plan.
  • Medicaid, VA, DoD, TRICARE and state programs: Lilly’s card excludes you. Coverage depends on your plan.

Lilly’s card ends on December 31, 2026, and Lilly can change it at any time. Our Mounjaro coupon guide quotes the terms in full, and our guide to whether insurance covers GLP-1s goes plan by plan.

How much is Mounjaro without insurance?

$499 a month at LillyDirect. To buy it, your prescriber sends the prescription to LillyDirect Pharmacy; a pharmacy partner then contacts you to confirm details and look for savings, and you choose home delivery or pickup at a retail pharmacy. LillyDirect says Mounjaro is FSA and HSA eligible, depending on your plan.

OptionHow it adds upYear total
Mounjaro self-pay, any dose13 × $499$6,487
Mounjaro with Lilly’s card, covered plan, 3-month fills at $254 fills of 3 months (48 weeks) + 1 month at $25 eachAs little as $125
Mounjaro list price, for reference13 × $1,112.16$14,458.08

The middle row is the best case. It assumes your plan allows 3-month fills and your share never goes past the card’s caps of $450 per 3-month fill and $1,950 a year. These are medication totals only. If an online program writes your prescription, its membership is extra.

Mounjaro for weight loss: why Zepbound costs less

Mounjaro and Zepbound both contain tirzepatide, made by Lilly, in the same six strengths. The difference is the label. Mounjaro is approved for blood sugar control in type 2 diabetes and to lower the risk of heart attack, stroke and cardiovascular death in adults with type 2 diabetes at high risk. The Zepbound label covers long-term weight reduction in adults with obesity, or overweight with a weight-related condition, and moderate to severe obstructive sleep apnea in adults with obesity. Prescribing Mounjaro for weight loss alone is off-label.

Zepbound is also cheaper to buy without insurance. LillyDirect sells Zepbound vials and the KwikPen at these self-pay prices:

Scroll sideways to see the full table

DoseZepbound, refill within 45 daysZepbound, refill after 45 daysMounjaro
2.5 mg$299$299$499
5 mg$399$399$499
7.5 mg$449$499$499
10 mg to 15 mg$449$699$499

Lilly says the $449 price at 7.5 mg and up comes from its Zepbound Journey Program and that regular prices of $499 at 7.5 mg and $699 at 10 mg to 15 mg apply if you don’t refill within 45 days. Vials need needles and syringes, and the KwikPen needs pen needles, bought separately. On a standard step-up with on-time refills, a year of Zepbound costs $299 + $399 + $449 + 10 × $449 = $5,637, against $6,487 for Mounjaro. Our Mounjaro vs Zepbound guide covers the trials and side effects, and our Zepbound cost guide covers each form.

Compounded tirzepatide costs less again: $85.58 to $378.08 a month at the programs we track, taking each program’s lowest ongoing price with medication and any required membership, as of October 2026. It is not Mounjaro or Zepbound, and not a generic of either. A compounding pharmacy prepares it, it isn’t FDA-approved, and the FDA says compounded drugs “are not reviewed by the agency for safety, effectiveness or quality prior to marketing” (FDA). Our compounded tirzepatide cost guide goes program by program.

What online programs charge for Mounjaro

Most programs set their own Mounjaro price, and several add a membership. As of October 2026:

ProgramMounjaro price it listsMembership
Mochi HealthAs low as $499 a monthBilled on top
FoundAbout $1,100 a monthPlan fee of $149 per 28-day cycle on top
Eden$1,399 a month$99 a month on top
Hims$1,899 a monthBilled separately
MeasuredInsurance requiredFrom $49 a month

A price far above $499 means the program isn’t passing on LillyDirect’s self-pay price. Ask which pharmacy fills the prescription and what you’ll pay at your dose before you sign up. Our tirzepatide provider rankings and brand-name GLP-1 rankings list the programs we track, in order of our published rating method.

Before you start

Mounjaro carries a boxed warning: in rats, tirzepatide caused thyroid C-cell tumors, and it is unknown whether it causes them, including medullary thyroid carcinoma (MTC), in people. It isn’t for anyone with a personal or family history of MTC or with MEN 2. Ask a licensed clinician whether it suits you before you compare prices.

Ways to pay less for Mounjaro

  • Check your coverage first. A covered fill with Lilly’s card can cost $25, and most Part D patients pay $50 or less, by Lilly’s figures.
  • Ask about a 3-month prescription if your plan allows it. Lilly’s card can cover all 3 months for as little as $25.
  • Paying cash, buy from LillyDirect at $499 rather than through a program charging $1,000 or more.
  • If the goal is weight loss, ask about Zepbound, which is approved for it and costs $299 to $449 a month self-pay with refills within 45 days.
  • Check prices again in January. Lilly’s card ends on December 31, 2026.

The bottom line

As of October 27, 2026, Mounjaro costs $499 a month self-pay at every dose, against a $1,112.16 list price. Commercial coverage with Lilly’s card can bring a fill to $25, and most Medicare Part D patients pay $0 to $50, according to Lilly. For weight loss, Mounjaro is off-label, and Zepbound costs less with on-time refills. Check your plan, then talk to a licensed clinician about which one fits your diagnosis. Questions, answered

How much is Mounjaro without insurance?

$499 a month for the single-dose pen at LillyDirect, at every dose from 2.5 mg to 15 mg, as of October 27, 2026. A month is 4 pens, so a year takes 13 fills and costs $6,487. If you have commercial insurance that doesn’t cover Mounjaro, Lilly’s savings card gives the same $499 price.

How much does Mounjaro cost with insurance?

It depends on your plan’s copay, coinsurance and deductible. If your commercial plan covers Mounjaro, Lilly’s savings card can bring a fill to $25, with savings capped at $150 a month and $1,950 a year. Lilly says about 8 in 10 Medicare Part D patients pay $0 to $50 for a 28-day supply.

Does the Mounjaro price go up with the dose?

Not at LillyDirect. The self-pay price is $499 a month at 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg, as of October 27, 2026. Lilly also quotes one list price, $1,112.16 per fill.

Is Mounjaro cheaper than Zepbound?

No, not if you refill Zepbound on time. Self-pay Zepbound vials and KwikPens cost $299 at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg to 15 mg with refills within 45 days. Mounjaro is $499 at every dose. Mounjaro costs less only than a late Zepbound refill at 10 mg to 15 mg ($699).

Does Medicare cover Mounjaro?

Lilly says Mounjaro is covered on many Medicare Part D plans for type 2 diabetes. The Medicare GLP-1 Bridge, the $50 program for weight loss, doesn’t include Mounjaro, and Lilly’s savings card excludes everyone on Medicare.
