Safety

One of these treatments carries an FDA boxed warning, the strongest warning on a label: semaglutide.

SemaglutideNot FDA-approved
GLP-1 medications carry a boxed warning for thyroid C-cell tumors and are not for people with a personal or family history of medullary thyroid carcinoma or MEN 2. FDA label

Not medical advice. A licensed clinician decides whether a treatment is right for you; read the full prescribing information before you start.

How GLP-1 weight loss medications work

GLP-1 medications copy a hormone your gut releases after you eat. They signal fullness to your brain and slow how quickly food leaves your stomach, so most people feel satisfied with less food and think about food less often. Many describe the quieter “food noise” as the biggest surprise.

Most are a once-weekly injection you give yourself at home with a short, thin needle. Two are now daily pills. Doses start low and step up every four weeks or so, which keeps the most common side effects, mainly nausea and other stomach upset, more manageable.

Brand-name or compounded?

Brand-name GLP-1s such as Wegovy and Zepbound are FDA-approved and made by their manufacturers. Compounded versions are prepared by state-licensed pharmacies for an individual prescription. They usually cost less, but they are not FDA-approved and the FDA does not review them for safety, effectiveness or quality. Our compounded GLP-1 guide covers what changed in 2025 and how to choose a program carefully.

What to look for in a provider

  • The price at your maintenance dose, not just month one. Programs that charge the same at every dose are easier to budget for.
  • A real clinical review. A licensed clinician should review your history and adjust your dose when side effects hit or progress stalls.
  • A licensed US pharmacy, and a clear statement of whether your medication is compounded or FDA-approved.
  • Terms you can live with. Check for prepaid terms, cancellation windows and what happens if you are not prescribed.

Questions, answered

What is GLP-1 microdosing?

Taking a GLP-1 at smaller weekly doses than the standard weight-loss schedule. The Wegovy and Zepbound labels start at 0.25 mg and 2.5 mg a week and step up from there; microdose plans stay lower, for example 0.2 mg of compounded semaglutide or 1.5 mg of compounded tirzepatide a week at one program we track. The weight-loss results on the FDA labels come from standard doses.

How much does a GLP-1 microdosing program cost?

Compounded microdose plans on this page run $99 to $189 a month at each program’s lowest ongoing rate, medication included; first-order and sale prices are left out. Some of those rates need a 3-month plan.

Is GLP-1 microdosing off-label?

For an FDA-approved drug, yes: prescribing Wegovy, the Wegovy pill or Zepbound below the dose schedule on its label is off-label use. Most microdose plans use compounded semaglutide or tirzepatide instead, which are not Wegovy or Zepbound and not generics of them. A compounding pharmacy prepares them, and they aren’t FDA-approved.

Why do people try GLP-1 microdosing?

Mostly in the hope of fewer side effects or a lower cost. No large trial has tested microdosing for weight loss, so the benefits and the right dose are unknown; the results on the FDA labels come from the standard dose ladders.

Who shouldn’t try GLP-1 microdosing?

The same people who shouldn’t take a GLP-1 at any dose: anyone with a personal or family history of medullary thyroid cancer or MEN 2, anyone pregnant or planning a pregnancy, and anyone with a history of pancreatitis should talk it through with a clinician first. A lower dose doesn’t remove the boxed warning.

Can I move from a microdose plan to a standard dose?

Yes, with your clinician. The standard schedules step up every 4 weeks, so a clinician can carry on from a low dose. Check whether your program prices the standard doses differently from its microdose plan.
