What it costs

  • 6 of the 6 providers here publish an all-in monthly starting price: $99 to $285 a month, with a median of $159. That includes the medication and any membership the provider requires.
  • 2 of these prices need a prepaid or fixed plan of 3 months or more; each says so beside the price.
  • A sale counts at its regular price, with the sale price shown beside it.

Every provider’s price, side by side: the longevity price index.

Safety

One of these treatments carries an FDA boxed warning, the strongest warning on a label: methylene blue.

Methylene blueCompounded; not FDA-approved
Boxed warning: serotonin syndrome with serotonergic drugs and opioids.
SermorelinCompounded; not FDA-approved
Compounded sermorelin isn’t FDA-reviewed for safety, effectiveness or quality, and its old Geref labeling covered children, not adults using it for aging.
NAD+Compounded; not FDA-approved
There is no FDA label, and compounded NAD+ isn’t FDA-reviewed for safety, effectiveness or quality.
GlutathioneCompounded; not FDA-approved
There is no FDA label for injectable glutathione, and compounded glutathione isn’t FDA-reviewed for safety, effectiveness or quality.
Vitamin B12 and lipotropic (MIC) injectionsCompounded; not FDA-approved
The cyanocobalamin label warns of anaphylaxis, which has been fatal, after injected B12; of low potassium when severe anemia is treated intensely; and of rapid optic nerve damage in people with early Leber’s disease.

How online longevity treatment works

Online longevity clinics sell treatments such as NAD+ injections and sermorelin, usually compounded and often with lab panels. The evidence behind many of them is limited, so we compare prices and clinical oversight and say plainly what each treatment is and isn’t approved for.

You fill in a health questionnaire on the provider’s own site. A licensed clinician reviews it, often with lab results, and decides whether a treatment is right for you. If you are prescribed, a licensed pharmacy ships it, and most providers renew the prescription on a subscription.

Treatment Active ingredient Form Status
NAD+ Nicotinamide adenine dinucleotide (NAD+) Subcutaneous injection or nasal spray Compounded; not FDA-approved
Sermorelin Sermorelin acetate (growth hormone-releasing hormone 1-29) Subcutaneous injection; also nasal spray, troche, lozenge or under-the-tongue tablet Compounded; not FDA-approved
Glutathione Glutathione (reduced, GSH) Injection; also nasal spray and rapid-dissolve tablet Compounded; not FDA-approved
Metformin for longevity Metformin hydrochloride Tablet (immediate or extended release) FDA-approved; off-label here
Rapamycin (sirolimus) Sirolimus Tablet (generic) or compounded capsule FDA-approved; off-label here
Low-dose naltrexone (LDN) Naltrexone hydrochloride Capsule, tablet or troche Compounded; not FDA-approved
Methylene blue Methylthioninium chloride Oral capsule (compounded) Compounded; not FDA-approved

FDA-approved or compounded?

FDA-approved products, including generics, are made by their manufacturers to an approved standard. Compounded products are prepared by state-licensed pharmacies for an individual prescription, often to combine ingredients or change the form. They are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality.

NAD+ injections and sermorelin are compounded and are not FDA-approved for anti-aging. Evidence for anti-aging benefits is limited.

What to look for in a provider

  • The monthly price for the treatment you want, and what it covers: the consult, refills and shipping.
  • A real clinical review. A licensed clinician should review your history before prescribing and order lab tests where the treatment calls for them.
  • A licensed US pharmacy, and a clear statement of which products are FDA-approved and which are compounded.
  • Terms you can live with. Check the subscription length, how to cancel and what happens if you are not prescribed.

Questions, answered

What is the best online longevity clinic?

It depends on the treatment you want. Our ranked list scores each clinic on all-in price, clinical care, treatment options, transparency and experience. Clinics that include medication, care and shipping in one monthly price score higher on value than those that add a membership on top, and clinics that include lab work score higher on care for treatments such as rapamycin.

Do longevity treatments actually work?

No treatment sold by these clinics has been shown to extend lifespan or slow aging in people. Rapamycin extends lifespan in mice, and a 48-week human trial found low weekly doses relatively safe but did not change its main outcome. NAD+ injections and sermorelin have only small, short human studies. Read is longevity medicine legit? for what is and isn’t proven.

How much do online longevity clinics cost?

The lowest ongoing price for NAD+ injections is Strut Health’s, at $149 a month. The lowest ongoing price for sermorelin is Strut Health’s, at $99 a month. Memberships at the clinics that require one run $99 a month, and every figure here already includes it. Each treatment page on this site lists the full range.

Are NAD+ and sermorelin FDA-approved?

No. Compounded NAD+ and sermorelin are not FDA-approved. A licensed pharmacy prepares them for your prescription. NAD+ is compounded under an interim FDA policy (Category 1 of its 503A list), and sermorelin was once sold as the approved drug Geref, which was discontinued in 2008. Rapamycin and metformin are FDA-approved drugs, but using them for aging is off-label.

Do online longevity clinics require lab work?

Some do. Rapamycin is usually prescribed after baseline labs, and a few clinics start every member with a blood panel; others prescribe NAD+ or sermorelin after a questionnaire alone. Each listing’s review says what it requires.

Is rapamycin safe to take for longevity?

Rapamycin (sirolimus) is an immunosuppressant, and its transplant label carries a boxed warning for serious infections and cancer at transplant doses. Clinics prescribe much lower, often weekly, doses off-label; small trials such as PEARL found no serious safety signal over about a year, but no proven benefit for aging.
