In short
- Zepbound costs $299 at 2.5 mg, $399 at 5 mg and $449 a month from 7.5 mg if you refill within 45 days, so a first year at LillyDirect comes to about $5,188.
- At self-pay prices, a first year costs about $3,888 on the Wegovy injection, $3,338 on the Wegovy pill and $3,188 on Foundayo, following each label’s dose schedule.
- In the head-to-head SURMOUNT-5 trial, people lost 20.2% of their weight on tirzepatide and 13.7% on semaglutide over 72 weeks.
- Compounded tirzepatide costs $125 to $399 a month at the 39 programs we rank (each program’s lowest ongoing price), but it isn’t Zepbound and isn’t FDA-approved.
- Mounjaro is the same drug as Zepbound, approved for type 2 diabetes; with diabetes, it is often the version insurance covers.
Zepbound (tirzepatide) produced the largest average weight loss of any approved weight-loss drug in its trials, but it isn’t cheap: self-pay from LillyDirect it costs $299 to $449 a month with on-time refills, and insurance coverage for weight loss is patchy. If the price, a coverage denial or a preference for pills has you looking elsewhere, you have real options. Here is how they compare, as of October 2026.
Why people look for an alternative
- Cost. At LillyDirect, Zepbound is $299 at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg to 15 mg if you refill within 45 days. A late refill at the higher doses costs $499 to $699.
- No coverage. Many plans exclude weight-loss drugs. Lilly’s savings card can bring a covered prescription to as little as $25 a month, but government insurance is excluded, and Lilly says its savings cards expire on December 31, 2026.
- Needles. Zepbound is a weekly injection; there is no tirzepatide pill.
- Side effects. Nausea and other stomach effects lead some people to try a different drug or a slower dose.

| Option | What it is | Average weight loss in its main trial | Self-pay price |
|---|---|---|---|
| Wegovy injection | Semaglutide, weekly | 14.9% at 2.4 mg (68 weeks); 18.8% at 7.2 mg (72 weeks) | $199 for the first two fills, then $349; $399 for 7.2 mg |
| Wegovy pill | Oral semaglutide, daily | 13.6% at 25 mg (64 weeks) | $149 to $299 by dose |
| Foundayo | Orforglipron, daily pill | 11.1% at 17.2 mg (72 weeks) | $149 to $299 by dose |
| Mounjaro | Tirzepatide for type 2 diabetes | Same drug as Zepbound; trials measured blood sugar | Usually through insurance |
| Liraglutide | Daily injection (Saxenda and generic) | 7.4% (56 weeks) | Pharmacy prices vary |
| Compounded tirzepatide | Not FDA-approved | Not studied in the approval trials | $125 to $399 a month |
For comparison, people in SURMOUNT-1 lost 15.0% (5 mg), 19.5% (10 mg) and 20.9% (15 mg) on Zepbound over 72 weeks, against 3.1% on placebo.
Wegovy: the closest match
The Wegovy injection is the nearest alternative in both effect and format. In the head-to-head SURMOUNT-5 trial of 751 adults, people on tirzepatide lost 20.2% of their body weight over 72 weeks, against 13.7% on semaglutide, so expect less on average. The gap narrows at Wegovy’s higher dose: in STEP UP, 7.2 mg produced 18.8% over 72 weeks. Wegovy also carries an approval Zepbound doesn’t: lowering the risk of heart attack, stroke and cardiovascular death in adults with heart disease and obesity or overweight.
Self-pay at NovoCare Pharmacy, the injection is $199 a month for the first two fills (for new patients through December 31, 2026), then $349 at every dose up to 2.4 mg. See Wegovy vs Zepbound for side effects and dosing side by side.
A pill instead of a shot
The Wegovy pill and Foundayo are the two approved weight-loss pills. Both start at $149 a month self-pay. The Wegovy pill has to be taken on an empty stomach with a small sip of water, 30 minutes before food or other medicines; Foundayo can be taken at any time, with or without food. Average weight loss in their main trials was 13.6% and 11.1%, lower than Zepbound’s, so a pill trades some effect for convenience and price. Our Foundayo vs Wegovy pill guide compares them.
Mounjaro: the same drug for diabetes
Mounjaro contains tirzepatide, the same medicine as Zepbound, and is approved for blood sugar control in type 2 diabetes. If you have type 2 diabetes, your plan may cover Mounjaro when it won’t cover Zepbound, and the effect on weight is the same drug’s. Without diabetes, prescribing Mounjaro for weight loss is off-label, and Zepbound is the version with self-pay pricing.
Compounded tirzepatide
Online programs sell compounded tirzepatide at one monthly price, usually below Zepbound’s. As of October 2026, it costs $125 to $399 a month at the 39 programs we rank, taking each program’s lowest ongoing monthly price with medication and any required membership included. Compounded tirzepatide is not Zepbound or Mounjaro, and not a generic of either. A compounding pharmacy prepares it, it isn’t FDA-approved, and the FDA doesn’t review compounded drugs for safety, effectiveness or quality.
The rules have tightened since the FDA declared the tirzepatide shortage resolved in late 2024. Pharmacies may now compound it only for an individual patient with a prescription, and not regularly or in large amounts when it is essentially a copy of an approved drug. Read compounded tirzepatide vs Zepbound, see what each program charges in how much compounded tirzepatide costs, and compare programs on our compounded tirzepatide list.
What a first year costs
This is our arithmetic from the self-pay prices, following each label’s dose schedule and assuming you reach the usual maintenance dose and refill on time. Your clinician may move you more slowly or stop at a lower dose. Some of these prices are limited-time offers that end December 31, 2026, including NovoCare’s $199 first fills of Wegovy and Lilly’s current self-pay offers, so a first year that runs into 2027 may cost more.
| Option | First-year cost | How we got there |
|---|---|---|
| Zepbound (LillyDirect) | About $5,188 | $299 + $399, then 10 months at $449 with on-time refills |
| Wegovy injection (NovoCare) | About $3,888 | 2 months at $199, then 10 at $349 |
| Wegovy pill (NovoCare) | About $3,338 | $149 + $199, then 10 months at $299 |
| Foundayo (LillyDirect) | About $3,188 | 2 months at $149, $199, then 9 months at $299 |
| Compounded tirzepatide | $1,500 to $4,788 | 12 months at $125 to $399, before any dose-based increases |
If your commercial plan covers a brand-name drug, a manufacturer savings card can bring it far below any of these. Medicare Part D enrollees who qualify can get Wegovy, the Zepbound KwikPen or Foundayo for $50 a month through the Medicare GLP-1 Bridge until December 31, 2027. See does Medicare cover Wegovy or Zepbound.
Before you switch
- Ask about dose. Zepbound and Wegovy use different dose ladders, so a switch usually means restarting partway up the ladder, with stomach side effects possible again.
- Check your coverage first. A denied Zepbound claim doesn’t mean every GLP-1 is excluded; plans sometimes cover one and not another.
- Look at your refill timing. If Zepbound’s cost is the problem only because of late-refill pricing, refilling within 45 days keeps the $449 price at the higher doses.
- Keep the safety warnings in mind. All of these GLP-1 medicines carry a boxed warning for thyroid C-cell tumors.
Online programs can prescribe any of these after a review. Compare them on our Zepbound programs, tirzepatide providers and GLP-1 pill programs lists, or start at the weight loss hub. If your reason is Ozempic rather than Zepbound, see Ozempic alternatives.
The bottom line
Zepbound costs about $5,188 for a first year at self-pay prices with on-time refills. The Wegovy injection is the closest alternative, with somewhat less weight loss on average and a lower price; the Wegovy pill and Foundayo cost less again and avoid needles; Mounjaro is the same drug if you have type 2 diabetes. Compounded tirzepatide is cheapest but isn’t FDA-approved. A licensed clinician can help you weigh effect, price and coverage.
