In short

  1. As of October 26, 2026, LillyDirect sells Foundayo for $149 a month at 0.8 mg and 2.5 mg, $199 at 5.5 mg, and $299 at 9 mg, 14.5 mg and 17.2 mg, self-pay. A month is 30 days.
  2. The 2.5 mg and 5.5 mg prices are temporarily reduced through December 31, 2026, from $199 and $299. At 14.5 mg and 17.2 mg, the $299 price needs a refill within 45 days; otherwise it’s $349.
  3. On the label’s dose steps, the first 12 fills come to $3,188 at today’s prices, or $3,338 at Lilly’s regular prices.
  4. With commercial coverage, Lilly’s savings card can bring a Foundayo fill to $25, with savings capped at $100 a month and $1,000 a year. Eligible Medicare Part D enrollees pay $50 a month through the GLP-1 Bridge.
  5. Foundayo’s list price is $649 a month, against $1,349.02 for the Wegovy pill. At maintenance doses, both cost $299 a month self-pay, with on-time refills at Foundayo’s top two doses.

As of October 26, 2026, Foundayo costs $149 to $299 a month if you pay cash at LillyDirect, depending on the dose. The 0.8 mg and 2.5 mg doses are $149, 5.5 mg is $199, and 9 mg, 14.5 mg and 17.2 mg are $299, the last two only if you refill within 45 days. Two of those prices are temporary cuts that run through December 31, 2026. With commercial coverage, Lilly’s savings card can bring a fill to $25, and eligible Medicare Part D enrollees pay $50.

Foundayo (orforglipron) is Lilly’s once-daily GLP-1 pill for weight loss. Below are the prices by dose, what a year adds up to, what insurance changes and how the cost compares with the Wegovy pill.

Foundayo price by dose

These are LillyDirect’s self-pay prices, read on October 26, 2026, next to the regular prices in Lilly’s offer terms. A month is a 30-day supply.

Dose (daily)When the label uses itLillyDirect price nowLilly’s regular price
0.8 mgFirst 30 days$149$149
2.5 mgNext 30 days or more$149 (reduced through Dec 31, 2026)$199
5.5 mgNext 30 days or more$199 (reduced through Dec 31, 2026)$299
9 mgIf your clinician steps you up$299$299
14.5 mgIf your clinician steps you up$299 with a refill within 45 days$349
17.2 mgMaximum dose$299 with a refill within 45 days$349

The Foundayo label starts you at 0.8 mg once daily, then 2.5 mg and 5.5 mg, each after at least 30 days on the dose before. From 5.5 mg, your clinician can move you to 9 mg, 14.5 mg or 17.2 mg, one step at a time and at least 30 days apart, based on how you respond and what you tolerate. The label caps the dose at 9 mg if you also take a strong CYP3A4 inhibitor, a type of drug interaction your clinician will check for.

Two conditions sit behind the prices (Lilly):

  • Temporary cuts. Lilly has cut 2.5 mg to $149 and 5.5 mg to $199 through December 31, 2026. Its regular prices for those doses are $199 and $299.
  • On-time refills at the top doses. At 14.5 mg and 17.2 mg, Lilly’s Self-Pay Journey Program gives you $299 automatically on your first fill, and you keep it only if each refill is bought within 45 days of the last delivery. A late refill costs $349.

Because 9 mg, 14.5 mg and 17.2 mg all cost $299 with on-time refills, moving up from 9 mg doesn’t change what you pay.

What a year of Foundayo costs

Each fill is 30 days, so 12 fills cover 360 days. Here is what the first 12 fills cost on the label’s steps, for the medication alone.

PathFirst 12 fillsTotal
0.8, 2.5 and 5.5 mg, then 9 mg or higher with on-time refills, at today’s prices$149 + $149 + $199 + 9 × $299$3,188
The same steps at Lilly’s regular prices$149 + $199 + $299 + 9 × $299$3,338

If you start now, your second and third fills fall before December 31, 2026, so you’d likely get both temporary cuts. Each late refill at 14.5 mg or 17.2 mg adds $50.

For context, Lilly lists Foundayo’s list price, the wholesale price before discounts, at $649 a month as of April 3, 2026 (Lilly). Self-pay buyers pay the LillyDirect prices instead.

How to buy Foundayo self-pay

Your prescriber sends the prescription to LillyDirect, and one of its dispensing pharmacies (Amazon Pharmacy, Fuze Health, Gifthealth or Walmart Pharmacy) contacts you to confirm details and apply savings. LillyDirect offers free home shipping or free pickup at a retail pharmacy.

If you fill somewhere else, Lilly’s Foundayo self-pay card gives the same prices, for up to 10 fills a calendar year. You agree not to bill any insurer, Medicare and Medicaid included, for a self-pay fill or count it toward a deductible. If the pharmacy can’t confirm an on-time refill at the counter, you can claim the difference back from Lilly until March 31, 2027.

Foundayo with insurance or Medicare

Commercial insurance that covers Foundayo. Lilly’s savings card lets you pay as little as $25 for a 1, 2 or 3-month fill. Savings are capped at $100 a month ($200 for 2 months, $300 for 3) and $1,000 a calendar year, for up to 10 fills, and the card ends on December 31, 2026. If your copay is $125 a month or less, you’d pay $25; above that, you pay the difference over $100. LillyDirect says it checks coverage and helps with prior authorization if your plan needs one.

Commercial insurance that doesn’t cover it. The same card gives you the self-pay prices above: $149, $149, $199 and $299 by dose.

Government insurance. Lilly’s card excludes anyone enrolled in Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, TRICARE or a state drug assistance program. Eligible Part D enrollees can instead get Foundayo for weight management through the Medicare GLP-1 Bridge, with a $50 copay from July 1, 2026 to December 31, 2027. CMS says the $50 doesn’t count toward your Part D out-of-pocket costs and the low-income subsidy doesn’t apply to it. Our Medicare guide explains who qualifies.

Foundayo vs the Wegovy pill on cost

The Wegovy pill is the other GLP-1 pill approved for weight loss, and its self-pay prices are close to Foundayo’s.

Commercial savings cardAs little as $25; up to $100 off a month and $1,000 a yearAs little as $25; up to $100 off a month
Medicare GLP-1 Bridge$50 a month$50 a month

Wegovy pill prices come from NovoCare, read October 26, 2026, where a month is one bottle of 30 tablets. On its label, the Wegovy pill steps up every 30 days from 1.5 mg to the 25 mg maintenance dose, and you take it on an empty stomach and wait at least 30 minutes before eating. Foundayo can be taken with or without food. The $150 gap over a first year comes entirely from Foundayo’s temporary cuts at months 2 and 3.

Price is close enough that the choice usually rests on other things, such as trial results and how each pill fits your routine. Our Foundayo vs Wegovy pill guide covers those, and our Wegovy pill cost guide has the Wegovy pill’s prices in full.

Questions, answered

How much is Foundayo a month without insurance?

At LillyDirect as of October 26, 2026: $149 at 0.8 mg and 2.5 mg, $199 at 5.5 mg and $299 at 9 mg, 14.5 mg and 17.2 mg. The 2.5 mg and 5.5 mg prices are reduced through December 31, 2026, and the $299 price at 14.5 mg and 17.2 mg needs a refill within 45 days.

Is Foundayo covered by insurance?

It depends on your plan. If your commercial plan covers it, Lilly’s savings card can bring a fill to $25, up to $100 off a month and $1,000 a year. Eligible Medicare Part D enrollees can get it for $50 a month through the GLP-1 Bridge. Medicaid, VA and TRICARE members can’t use Lilly’s card.

Is Foundayo cheaper than the Wegovy pill?

Slightly, in the first months. Both start at $149, but Foundayo’s second and third steps cost $149 and $199 against the Wegovy pill’s $199 and $299, through December 31, 2026. At maintenance, both are $299 a month self-pay.

Why does a higher Foundayo dose cost the same?

Lilly charges $299 a month for 9 mg, 14.5 mg and 17.2 mg, as long as you refill 14.5 mg or 17.2 mg within 45 days. Moving up a dose from 9 mg doesn’t raise the self-pay price unless a refill is late.
