In short

  1. The Ozempic pill is oral semaglutide in 1.5, 4 and 9 mg tablets, approved for type 2 diabetes. It shares one FDA label and application (NDA 213051) with Rybelsus, which NovoCare says is being replaced by the Ozempic pill.
  2. When you switch, Rybelsus 7 mg matches Ozempic tablets 4 mg and Rybelsus 14 mg matches 9 mg. The label says the two aren’t interchangeable milligram for milligram.
  3. Novo lists the Ozempic pill at $149 a month for 1.5 mg, $199 for 4 mg and $299 for 9 mg self-pay, read October 27, 2026. With commercial coverage, its savings offer can bring a fill to as little as $25.
  4. The Ozempic pill and Rybelsus aren’t approved for weight loss, so prescribing them for that alone is off-label. The Wegovy pill is the oral semaglutide approved for weight loss, at doses up to 25 mg.
  5. Both carry a boxed warning for thyroid C-cell tumors. In the label’s placebo-controlled trials, nausea affected 11% to 20% of people, against 6% on placebo.

The Ozempic pill is Novo Nordisk’s once-daily semaglutide tablet for adults with type 2 diabetes, sold in 1.5, 4 and 9 mg. It shares an FDA label with Rybelsus, the older semaglutide tablet it is replacing, and it isn’t approved for weight loss: that’s the Wegovy pill.

Self-pay prices run from $149 to $299 a month by dose, based on Novo’s savings page read October 27, 2026. Below is how the Ozempic pill relates to Rybelsus, the Ozempic injection and the Wegovy pill, how to take it, and what it costs.

What the Ozempic pill is, and what it isn’t

Ozempic tablets and Rybelsus are listed together on one FDA application, NDA 213051, and share one prescribing label, revised January 2026. Both are approved, with diet and exercise, to improve blood sugar in adults with type 2 diabetes, and to lower the risk of heart attack, stroke and cardiovascular death in adults with type 2 diabetes at high risk of those events.

On its Rybelsus page, NovoCare says plainly that “RYBELSUS is being replaced by Ozempic pill,” and that your clinician will guide the change (NovoCare). Novo’s own Ozempic pill FAQ says the pill “is not a weight-loss drug” (Ozempic).

All four contain semaglutide, but they aren’t interchangeable. Each has its own dose schedule, and none should be swapped for another without a licensed clinician.

Rybelsus vs Ozempic: what changed

The tablets differ in how much semaglutide gets into your blood. Both use an absorption enhancer called SNAC, and absorption happens mainly in the stomach. The label puts absolute bioavailability at about 0.4% to 1% for Rybelsus and 1% to 2% for Ozempic tablets. In a study in healthy volunteers, blood levels on Rybelsus 3, 7 and 14 mg showed no clinically significant difference from Ozempic tablets 1.5, 4 and 9 mg. That’s why the milligrams differ.

RybelsusOzempic pill
3 mg (starting dose; no switching in the first 30 days)1.5 mg (starting dose)
7 mg4 mg
14 mg9 mg

The label’s switching rules:

  • Don’t switch during the first 30 days on either tablet.
  • After that, start the other tablet the day after your last dose, at the matching strength above.
  • You need a new prescription to move from Rybelsus to the Ozempic pill, according to NovoCare, which also says the Ozempic pill has “the same coverage as RYBELSUS.”

Rybelsus or the Ozempic pill vs the Ozempic injection

The Ozempic injection is weekly, the tablets daily. The label allows people on the 0.5 mg Ozempic injection to switch to tablets: start Ozempic tablets 4 mg or 9 mg, or Rybelsus 7 mg or 14 mg, one week after the last injection. The injection goes up to 2 mg and has its own approved uses, so a clinician decides which form fits.

How to take the Ozempic pill

The tablet has strict food and water rules, the same as Rybelsus:

  1. Take it once a day in the morning on an empty stomach.
  2. Swallow it whole with up to 4 ounces of plain water. Don’t split, crush or chew it, and don’t take it with other drinks.
  3. Wait at least 30 minutes before eating, drinking or taking other oral medicines.

The first-month dose isn’t effective for blood sugar control. It’s there to help your stomach adjust.

Rybelsus and the Ozempic pill for weight loss

Neither tablet is approved for weight loss, so a prescription for weight loss alone is off-label. The label’s trials measured blood sugar, and weight changes were modest. In a 26-week trial of 703 adults with type 2 diabetes, average weight fell 3.7 kg (about 8 pounds) on Rybelsus 14 mg and 2.3 kg on 7 mg, against 1.4 kg on placebo, from a starting weight of about 88 kg.

The oral semaglutide approved for weight loss is the Wegovy pill. In its main 64-week trial, people without diabetes on 25 mg lost 13.6% of their body weight on average, against 2.4% on placebo (Wegovy label). It follows the same empty-stomach routine, and NovoCare lists it at $149 for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg (NovoCare, read October 27, 2026). The other approved weight-loss pill is Foundayo, which you can take with or without food. Our Foundayo vs Wegovy pill guide compares the two.

Ozempic pill price, Rybelsus cost and coupons

Novo’s self-pay prices for the Ozempic pill, read October 27, 2026:

DoseSelf-pay per month
1.5 mg$149 (new patients)
4 mg$199
9 mg$299

A month is one bottle of 30 tablets, and Novo says it can change or end the program at any time.

With commercial insurance. If your plan covers it, Novo’s savings offer can bring any dose to as little as $25 for up to a 3-month prescription, with savings capped at $100 a month. People with government insurance are excluded. NovoCare says people already using a Rybelsus savings offer can keep using it for the Ozempic pill without signing up again.

Rybelsus cost. NovoCare’s Rybelsus page lists no separate self-pay price; it points Rybelsus patients to the Ozempic pill’s self-pay pricing. In February 2026, Novo announced it would cut the list price of Rybelsus 7 mg and 14 mg, along with Ozempic and Wegovy, to $675 a month from January 1, 2027, and said self-pay prices won’t change (Novo Nordisk).

The injection, for comparison. New self-pay patients pay $199 a month for the first two fills of the Ozempic pen at 0.25 mg or 0.5 mg through December 31, 2026, then $349 a month for 0.25, 0.5 or 1 mg and $499 for 2 mg. Our Ozempic cost guide and Ozempic coupon guide go further.

Side effects and safety

Boxed warning. Semaglutide caused thyroid C-cell tumors in rodents, and it isn’t known whether it does in people. The Ozempic pill and Rybelsus aren’t for anyone with a personal or family history of medullary thyroid carcinoma or MEN 2.

The most common side effects in the label’s two placebo-controlled trials, at the Rybelsus doses that match Ozempic tablets 4 mg and 9 mg:

Nausea6%11%20%
Abdominal pain4%10%11%
Diarrhea4%9%10%
Decreased appetite1%6%9%
Vomiting3%6%8%
Constipation2%6%5%

Most nausea, vomiting and diarrhea came while the dose was rising. Stomach side effects led 8% of people on 14 mg and 4% on 7 mg to stop, against 1% on placebo. The label’s other warnings cover pancreatitis, diabetic eye complications, low blood sugar when combined with insulin or a sulfonylurea, kidney injury from dehydration, gallbladder problems, serious allergic reactions and food entering the lungs under anesthesia. Stop at least 2 months before a planned pregnancy, and the tablets aren’t recommended while breastfeeding or with severe gastroparesis.

Getting it prescribed

Your regular doctor can prescribe the Ozempic pill for type 2 diabetes, and so can online clinicians. If your goal is weight loss without diabetes, ask about the approved options instead. As of November 2026, 5 of the 26 programs we rank prescribe the Wegovy pill, and our oral GLP-1 rankings compare programs that offer pills.

Compounded semaglutide tablets are a different product

Some online programs sell compounded semaglutide tablets or lozenges. At the 5 of the 26 programs we rank that publish a price, compounded tablets and lozenges cost $99 to $302.25 a month as of November 2026, such as Strut Health’s lozenge from $99 a month on auto-refill and MyDrHank from $171 a month. A compounded semaglutide tablet is not the Ozempic pill and not a generic of it. A compounding pharmacy prepares it, and it isn’t FDA-approved; the FDA doesn’t review compounded drugs for safety, effectiveness or quality before they’re sold (FDA).

The bottom line

The Ozempic pill is Rybelsus’s successor: the same daily semaglutide tablet for type 2 diabetes, in new strengths (1.5, 4 and 9 mg) that absorb better. It costs $149 to $299 a month self-pay, or as little as $25 with commercial coverage. It isn’t approved for weight loss. If that’s your goal, ask a licensed clinician about the Wegovy pill or Foundayo, which are.

Questions, answered

Is there an Ozempic pill?

Yes. Ozempic tablets come in 1.5, 4 and 9 mg and are taken once a day for type 2 diabetes. They are on the same FDA label as Rybelsus, and Novo Nordisk is moving Rybelsus patients to them. The weekly Ozempic injection is still sold.

Is Rybelsus the same as the Ozempic pill?

Both are semaglutide tablets from Novo Nordisk on one label, with the same uses and food rules, but the doses differ. The Ozempic pill absorbs better (about 1% to 2% vs 0.4% to 1%), so 4 mg matches Rybelsus 7 mg and 9 mg matches 14 mg. You need a new prescription to switch.

Can you take Rybelsus or the Ozempic pill for weight loss?

Neither is approved for weight loss, so using either for that alone is off-label. In a 26-week diabetes trial, Rybelsus 14 mg lowered weight by 3.7 kg on average, against 1.4 kg on placebo. The Wegovy pill is the oral semaglutide approved for weight management.

Is there a Rybelsus coupon?

Novo Nordisk’s savings offer can bring a commercially insured fill of the Ozempic pill to as little as $25 for up to a 3-month prescription, with savings capped at $100 a month. NovoCare says an existing Rybelsus savings offer keeps working for the Ozempic pill. Government plan members are excluded.

How much does Rybelsus cost?

NovoCare points Rybelsus patients to the Ozempic pill’s self-pay prices: $149 a month at 1.5 mg, $199 at 4 mg and $299 at 9 mg, read October 27, 2026. Novo plans to cut the list price of Rybelsus 7 mg and 14 mg to $675 a month on January 1, 2027.
