In short
- Blue Cross Blue Shield coverage of Wegovy depends on your regional company and employer: Massachusetts excludes it at renewals from January 1, 2026, while FEP Blue covers it with prior approval.
- Where plans cover weight loss, criteria look alike: a BMI of 30, or 27 with a weight-related condition, a diet or weight program, and at least 5% weight loss to renew after 5 to 8 months.
- CVS Caremark has preferred Wegovy on its commercial template formularies since July 1, 2025, and Aetna’s 2026 exclusion list names Wegovy as a covered option.
- Wegovy is also approved to cut heart attack and stroke risk (since March 8, 2024) and for MASH liver disease, and Aetna, Cigna and UnitedHealthcare review those uses on separate criteria.
- The Wegovy pill is harder to get covered: UnitedHealthcare’s October 2026 policy says Wegovy tablets are typically excluded, though FEP Blue and Aetna have criteria for them.
- With commercial coverage, the pen or pill can cost as little as $25 a month (up to $100 off a month); self-pay starts at $149 a month for the 1.5 mg pill.
Some Blue Cross Blue Shield plans cover Wegovy with prior authorization and others exclude it, because each regional Blue company and each employer sets its own rules. Blue Cross Blue Shield of Massachusetts excludes Wegovy at plan renewals from January 1, 2026, for example, while FEP Blue, the plan for federal employees, covers it for members who meet its criteria.
The same pattern holds at Aetna, Cigna and UnitedHealthcare: when your plan includes weight-loss drugs, Wegovy is usually covered after prior authorization. With coverage and Novo Nordisk’s savings offer, the pen or pill can cost as little as $25 a month (NovoCare).
Wegovy coverage by insurer
| Insurer or plan | Wegovy pen for weight loss | Wegovy pill |
|---|---|---|
| Blue Cross Blue Shield | Varies by Blue company and employer | Varies |
| FEP Blue (federal employees) | Covered with prior approval | Covered with prior approval |
| Aetna | Covered with prior authorization if your plan includes weight-loss drugs | Prior authorization criteria published |
| Cigna | Covered with prior authorization if your plan includes weight-loss drugs | Check your plan |
| UnitedHealthcare | Only if your employer elected weight-loss coverage | Typically excluded |
| Medicare | GLP-1 Bridge, $50 a month | GLP-1 Bridge, $50 a month |
Coverage differs even inside one insurer. The rules below come from each company’s standard policies on the dates shown; your plan’s drug list and benefit booklet decide.
Does Blue Cross Blue Shield cover Wegovy?
Blue Cross Blue Shield is a group of separately run regional companies, and employers that self-fund their plans make their own choices on top. Three Blue plans show the range:
- Blue Cross Blue Shield of Massachusetts excludes Wegovy, Saxenda and Zepbound at renewals starting January 1, 2026, and every GLP-1 use except type 2 diabetes, so the heart-risk use is excluded too. Its FAQ says the exclusion can’t be appealed, though employers with more than 100 employees can buy coverage back.
- Blue Cross Blue Shield of Michigan and Blue Care Network ended GLP-1 weight-loss coverage for fully insured large group members on January 1, 2025, or at a later renewal (provider alert). Self-funded groups could keep covering it.
- FEP Blue covers Wegovy under a policy effective February 13, 2026. Adults need a BMI of 30, or 27 with heart disease or a weight-related condition, plus a weight-management program. The first approval lasts 6 months, and renewal needs at least 5% weight loss. Unlike FEP’s Zepbound policy, it doesn’t ask you to try oral weight-loss drugs first.
If your Blue plan comes through work, ask HR whether it’s fully insured or self-funded and whether it covers weight-loss drugs, then check the drug list for Wegovy.
Does Aetna cover Wegovy?
Yes, with prior authorization, when your plan includes weight-loss drugs. Aetna’s 2026 exclusion list for its Standard Control Plan names Wegovy as a covered option in place of excluded weight-loss drugs. Its Wegovy policy asks adults for:
- At least 6 months in a weight-management program with diet, activity and behavior change before starting the drug
- A BMI of 30 or higher, or 27 or higher with a weight-related condition such as high blood pressure, type 2 diabetes or high cholesterol
The first approval lasts 8 months for the pen and Wegovy HD and 6 months for the pill. To renew for 12 months, you need at least 3 months at a stable maintenance dose and at least 5% weight loss, or to have kept that loss off.
Cigna, UnitedHealthcare and CVS Caremark
Cigna requires prior authorization under its Wegovy injection policy, reviewed June 17, 2026. Adults need 3 months of behavior change and a reduced-calorie diet and a BMI of 30, or 27 with a weight-related condition. The first approval lasts 8 months; after that, approvals run a year at a time if you’ve lost at least 5% of your starting weight. Evernorth, Cigna’s health services division, also offers plans a benefit option that caps the Wegovy or Zepbound copay at $200 a month.
UnitedHealthcare covers Wegovy only for employers that elect its weight-loss program, effective October 1, 2026. It uses the same BMI rules, approves Wegovy for 5 months at first, and renews for 12 months after at least 5% weight loss. If you haven’t reached a maintenance dose, it allows one extra 6-month approval.
CVS Caremark has preferred Wegovy on its commercial template formularies since July 1, 2025. It added Zepbound back as a second preferred option on October 1, 2026 (CVS Health), so Wegovy stays on those lists.
Heart risk and MASH: coverage beyond weight loss
Wegovy carries a boxed warning: in rodents, semaglutide causes thyroid C-cell tumors, and Wegovy isn’t for people with a personal or family history of medullary thyroid carcinoma or MEN 2. Its label covers more than weight loss:
- Heart risk. Wegovy injection and tablets lower the risk of cardiovascular death, heart attack and stroke in adults with established heart disease and obesity or overweight. The FDA approved this use on March 8, 2024.
- MASH. Wegovy injection treats noncirrhotic metabolic dysfunction-associated steatohepatitis with moderate to advanced liver scarring (stage F2 to F3) in adults, under accelerated approval.
Plans often review these uses separately. Aetna and Cigna approve the heart-risk use for 12 months with a prior heart attack, stroke or symptomatic peripheral artery disease and a BMI of at least 27; Aetna also lists past revascularization and excludes people with type 2 diabetes, who it points to Ozempic. For MASH, Aetna wants a gastroenterologist or hepatologist involved and fibrosis confirmed by imaging or biopsy. UnitedHealthcare points plans that exclude weight-loss drugs to separate Wegovy criteria for these uses.
Public programs follow the same split. Medicare sends heart-risk and MASH requests to your regular Part D plan rather than the Bridge (CMS form), and Medicaid must cover Wegovy for cardiovascular disease even where obesity coverage is optional (KFF).
Insurance coverage for the Wegovy pill
The Wegovy pill is approved for weight management and heart-risk reduction in adults (label), but coverage lags the pen:
- UnitedHealthcare’s October 2026 policy says Wegovy tablets “are typically excluded from coverage.”
- FEP Blue covers them with prior approval, up to 90 tablets per 90 days.
- Aetna’s Wegovy policy includes the pill, with a 6-month first approval.
- The Medicare GLP-1 Bridge lists Wegovy tablets alongside the injection and HD.
With commercial coverage, NovoCare’s savings offer applies to the pill too. Without coverage, NovoCare charges $149 a month for 1.5 mg, $199 for 4 mg and $299 for 9 mg and 25 mg (NovoCare). Our Wegovy pill cost guide and oral GLP-1 list go further.
How to get Wegovy approved, or appeal a denial
Before your clinician files, gather your starting BMI, any weight-related conditions, records of a diet or weight-management program, and, for heart risk or MASH, the cardiology or liver records. If the plan says no:
- Read the denial letter. Missing criteria can be fixed; a plan exclusion usually can’t.
- Resubmit with what was missing, or ask for a formulary exception if Wegovy isn’t on your drug list.
- File an internal appeal within 180 days of the denial (HealthCare.gov).
- Request an external review within 4 months if the plan still says no. Your insurer must accept the reviewer’s decision (HealthCare.gov).
Medicare and Wegovy
The Medicare GLP-1 Bridge runs from July 1, 2026, to December 31, 2027. Eligible Part D enrollees pay $50 a month for Wegovy injection, tablets or HD for weight loss, after a prior authorization their clinician files. See our Medicare guide.
What Wegovy costs with or without coverage
With commercial coverage, NovoCare says you can pay as little as $25 a month, with savings capped at $100 a month; people in government programs are excluded. Without coverage, new patients pay $199 a month for the first 2 months at 0.25 mg and 0.5 mg through December 31, 2026, then $349 a month for 0.25 mg to 2.4 mg and $399 for Wegovy HD 7.2 mg (NovoCare). Our Wegovy cost guide adds up a year.
Some online programs handle the insurance work. Among the 26 programs we rank, as of November 2026, Measured runs a benefits check and files prior authorizations for brand-name GLP-1s (membership from $49 a month, not billed to insurance), Found accepts most major insurers for clinician visits, and Sesame’s providers help with prior authorization. See GLP-1 programs that help with insurance and brand-name GLP-1 programs.
The bottom line
Whether BCBS, Aetna, Cigna or UnitedHealthcare covers Wegovy comes down to your plan: does it include weight-loss drugs, and what does its prior authorization ask for? If weight loss is excluded but you have heart disease or MASH, ask whether those criteria apply. Talk with a licensed clinician about which option fits you before you file.
