In short
- Phentermine is a Schedule IV controlled substance, labeled only as a short-term aid of ‘a few weeks’ for adults with a BMI of 30 or more, or 27 or more with another risk factor such as diabetes.
- Through December 31, 2026, a temporary DEA and HHS rule (90 FR 61301) lets DEA-registered clinicians prescribe Schedule II to V drugs, phentermine included, by telemedicine without an in-person visit first.
- The usual dose is one 37.5 mg tablet or capsule a day, before breakfast or 1 to 2 hours after. Lomaira is an 8 mg tablet, usually taken three times a day.
- Qsymia, phentermine plus topiramate, is labeled for long-term use. At the top dose, adults lost 9.8% to 10.9% of their weight on average over a year, against 1.2% to 1.6% on placebo.
- As of October 27, 2026, Qsymia costs $74.99 for 30 days through its home delivery pharmacy, and the telehealth partner its maker links to charges $65 for the consultation.
Yes, you can get phentermine prescribed online in 2026, after a live telemedicine visit with a clinician who is registered with the DEA. Phentermine is a Schedule IV controlled substance, and online prescribing without a prior in-person visit is allowed only under a temporary federal rule that runs through December 31, 2026.
Phentermine is also labeled for short-term use, “a few weeks,” which sets it apart from GLP-1 medications and from Qsymia, its long-term combination with topiramate. Below is what the rules allow, what the label says about doses and safety, and what an online prescription costs.
What is phentermine?
Phentermine is a stimulant appetite suppressant, a “sympathomimetic amine anorectic,” first approved in 1959. It’s chemically and pharmacologically related to amphetamine, which is why it’s a controlled substance.
The label approves it as a short-term add-on, “a few weeks,” to a weight-loss plan built on exercise, behavior change and calorie restriction. It’s for adults with a starting BMI of 30 or more, or 27 or more with another risk factor such as controlled high blood pressure, diabetes or high cholesterol. The label is modest about results: the extra weight lost on these drugs compared with placebo, in relatively short trials, was “only a fraction of a pound a week,” and tolerance to the appetite effect usually develops within a few weeks. If it does, the label says to stop rather than raise the dose.
The DEA rule behind online phentermine
Under the Ryan Haight Act of 2008, a clinician normally has to see you in person at least once before prescribing a controlled substance remotely. The DEA first waived that requirement in 2020, during the COVID-19 public health emergency, and has kept the waiver going with HHS through a series of temporary rules. The current one, the Fourth Temporary Extension, was published December 31, 2025 (90 FR 61301).
From now through December 31, 2026, a DEA-registered practitioner can prescribe Schedule II to V drugs, phentermine included, by telemedicine without an in-person evaluation, as long as:
- the prescription is for a legitimate medical purpose, in the usual course of practice;
- it follows a live visit over an interactive telecommunications system (audio and video for phentermine);
- the clinician’s DEA registration covers that class of drug; and
- it meets every other federal prescription rule, plus your state’s law.
What changes on January 1, 2027 depends on the DEA. If it doesn’t extend the rule again or finalize a permanent one, the in-person requirement returns for new patient relationships. It doesn’t apply once a clinician has seen you in person once, even years earlier. The DEA proposed a “special registration” for telemedicine prescribing in January 2025 (90 FR 6541), and as of October 27, 2026 the Federal Register shows no final rule or further extension.
How an online phentermine prescription works
Online programs usually follow the same path: a health questionnaire, a live visit with a licensed clinician, then a prescription sent to a pharmacy or shipped to you. Expect questions about your heart and blood pressure, thyroid, glaucoma, agitation and any history of drug misuse, since the label rules phentermine out with each. The label also asks prescribers to give “the least amount feasible” at one time, to limit the risk of overdose.
Some programs sell compounded phentermine rather than the FDA-approved generic. Henry Meds, for example, lists compounded phentermine tablets at $149 a month, including provider visits, medication and shipping, read October 27, 2026. A compounding pharmacy prepares it, and it isn’t FDA-approved; Henry Meds itself says compounded medications may differ in efficacy, safety and side effects from FDA-approved drugs. Ask any program which version it prescribes.
Phentermine 37.5 mg and other doses
| Product | Strength | Usual adult dose |
|---|---|---|
| Phentermine hydrochloride tablets or capsules | 37.5 mg (30 mg phentermine base) | One a day, before breakfast or 1 to 2 hours after |
| Half tablet | 18.75 mg | Once or twice a day, if the full dose isn’t needed |
| Lomaira | 8 mg | Usually one tablet three times a day, 30 minutes before meals |
The label says to use the lowest dose that works and to avoid taking it late in the day, which can cause insomnia. With severe kidney disease, the maximum is 15 mg a day. Lomaira’s label adds that it isn’t recommended for anyone 16 or younger.
Side effects and who shouldn’t take it
The phentermine label lists side effects without trial percentages: a faster heartbeat, palpitations and higher blood pressure; restlessness, dizziness, insomnia, tremor and headache; and dry mouth, an unpleasant taste, diarrhea or constipation. Rare but serious risks include primary pulmonary hypertension, a lung condition that can be fatal, and damage to the heart valves.
The label rules phentermine out with:
- a history of heart disease, including coronary artery disease, stroke, irregular heart rhythms, heart failure or uncontrolled high blood pressure;
- an MAOI antidepressant taken within the past 14 days;
- an overactive thyroid or glaucoma;
- agitated states or a history of drug misuse;
- pregnancy or breastfeeding.
It hasn’t been shown safe or effective combined with other weight-loss drugs, including over-the-counter and herbal products.
Qsymia weight loss pills
Qsymia is a once-daily capsule of phentermine with extended-release topiramate. It’s also Schedule IV, but unlike phentermine alone, its label covers long-term weight management for adults and teens 12 and older with obesity, and adults with overweight and at least one weight-related condition.
You start at 3.75 mg/23 mg for 14 days, then take 7.5 mg/46 mg. After 12 weeks, if an adult hasn’t lost at least 3%, the dose can step up to 11.25 mg/69 mg and then 15 mg/92 mg. If you haven’t lost 5% after 12 weeks at the top dose, the label says to stop, tapering over at least a week to avoid a seizure.

| One-year trial | Placebo | Lower dose | Qsymia 15 mg/92 mg |
|---|---|---|---|
| Study 1: BMI 35 or more | 1.6% | 5.1% (3.75 mg/23 mg) | 10.9% |
| Study 2: BMI 27 to 45 with 2 or more conditions | 1.2% | 7.8% (7.5 mg/46 mg) | 9.8% |
Topiramate can cause birth defects such as cleft lip and palate. So the label recommends a negative pregnancy test before starting and every month on treatment, plus effective birth control, and Qsymia is sold only through pharmacies certified under an FDA safety program (REMS). The most common side effects at the top dose were tingling (20% vs 2% on placebo), dry mouth (19% vs 3%) and constipation (16% vs 6%). It isn’t for anyone who is pregnant or has glaucoma or an overactive thyroid, or who took an MAOI in the past 14 days.
What phentermine and Qsymia cost online
Qsymia’s maker, VIVUS, publishes its cash prices (Qsymia and Qsymia Engage, read October 27, 2026):
| How you buy Qsymia | Price |
|---|---|
| Home delivery pharmacy, 30-day supply | $74.99, plus shipping |
| Home delivery pharmacy, 90-day prescription | $70 a month, where 90-day fills are allowed |
| Retail pharmacy with the manufacturer discount | $75 for 30 days |
| Costco pharmacy, members | $65.99 for 30 days |
| Telehealth consultation through its linked partner | $65 |
One older panel on the Qsymia Engage page still shows $89 for 30 days, so confirm the price at checkout. The home delivery program is cash only, and purchases can’t be submitted to Medicare or other government plans. The retail discount card isn’t available to people on government plans such as Medicare, Medicaid or TRICARE, or to residents of California and Massachusetts. Generic phentermine prices vary by pharmacy and dose, so we don’t quote one. Online programs set their own fees for the visit, so check whether the price includes the medication.
Phentermine vs GLP-1 medications
GLP-1 medications aren’t controlled substances (DailyMed lists no DEA schedule for Wegovy, for example), so the DEA rule doesn’t affect them, and they’re labeled for long-term use. Trial results come from separate studies, so treat the numbers as a rough guide, not a head-to-head comparison.

| Medication | Controlled | Labeled use | Average loss in its main trial | Price, as of Oct 2026 |
|---|---|---|---|---|
| Phentermine | Schedule IV | A few weeks | “A fraction of a pound a week” over placebo | Varies by pharmacy |
| Qsymia 15 mg/92 mg | Schedule IV | Long term | 9.8% vs 1.2% (1 year) | $74.99 a month |
| Contrave | No | Long term | 5.4% vs 1.3% (56 weeks) | $99 a month by mail |
| Wegovy pill 25 mg | No | Long term | 13.6% vs 2.4% (64 weeks) | $149 to $299 a month |
| Compounded semaglutide | No | Not FDA-approved | Not part of the brand trials | $65 to $299 a month |
Contrave’s figures come from its label and the Wegovy pill’s from the Wegovy label, with prices from Contrave’s maker and NovoCare, read October 27, 2026. Compounded semaglutide costs $65 to $299 a month at the 20 of the 26 programs we rank that publish a price, as of November 2026. It is not Wegovy and not a generic of it; a compounding pharmacy prepares it, and it isn’t FDA-approved.

The bottom line
You can get phentermine online through December 31, 2026, after a live video visit with a DEA-registered clinician, under a temporary rule that may or may not be extended. Phentermine is a Schedule IV stimulant labeled for a few weeks, with modest added weight loss and real heart and blood pressure cautions. Qsymia and the GLP-1s are labeled for long-term use. A licensed clinician can tell you which, if any, fits your health history.
