In short
- Daily generic tadalafil started at $40 a month at Hims, and at $2 a tablet during Rex MD’s fall sale, as of October 18, 2026.
- As-needed generic tadalafil cost $3 to $7.25 a tablet at the providers we checked; Strut Health’s 5 mg dose was $30 for 10 doses a month.
- TRT Nation sold 30 tadalafil 20 mg tablets for $99.99, about $3.33 a tablet.
- Brand-name Cialis cost from $29 a dose at Rex MD, and $958 a month for daily use at Hims.
- Tadalafil is also approved for an enlarged prostate (BPH), which is why some insurance plans cover the daily 5 mg dose.
Generic tadalafil, the FDA-approved form of Cialis’s active ingredient, is sold two ways. Daily tadalafil is a low dose, 2.5 mg or 5 mg, taken every day; it started at $40 a month at Hims on October 18, 2026, and at $2 a tablet during Rex MD’s fall sale. As-needed tadalafil is a higher dose, usually 10 mg or 20 mg, taken before sex; it cost $3 to $7.25 a tablet at the providers we checked.
Which is cheaper for you depends on how often you would take it. Below are each provider’s published prices, what they cover, and how the two ways of taking tadalafil compare on cost.
Daily or as needed: what the label says
The Cialis label approves two ways of taking it for ED:
- As needed: 10 mg before sex for most men, adjusted to 5 mg or 20 mg, no more than once a day. The label says it improved erectile function for up to 36 hours after a dose.
- Once daily: 2.5 mg at about the same time every day, raised to 5 mg if needed, without timing it to sex.
Daily 5 mg is also approved for symptoms of benign prostatic hyperplasia (BPH), an enlarged prostate, alone or together with ED. Food does not change how tadalafil is absorbed. Our tadalafil page covers dosing and safety.
Daily tadalafil prices
| Provider | Price | What it covers |
|---|---|---|
| Hims | From $40 a month | Medication, check-ins, messaging, free shipping |
| Rex MD | From $2 a tablet (fall sale) | Free consult, free rush shipping |
| Lemonaid Health | $30 for the first monthly shipment | Plus a one-time $25 consult; regular price not stated |
| Keeps | From $4.80 a dose (daily or as needed) | Free consult; billed every 3, 6 or 12 months |
At Rex MD’s sale price, 30 daily tablets would cost $60 a month; the page doesn’t say when the sale ends. Lemonaid’s $30 covers only the first shipment. Hims also lists generic tadalafil from $24 a month without saying whether that plan is daily or as needed. See our daily tadalafil rankings.
As-needed tadalafil prices
| Provider | Price | Price per tablet |
|---|---|---|
| Strut Health | $30 for 10 doses of 5 mg a month | $3 (5 mg), $6 (10 mg), $12 (20 mg) |
| TRT Nation | $99.99 for 30 tablets of 20 mg | About $3.33 |
| Keeps | From $4.80 a dose | $4.80 |
| Rex MD | From $6 a tablet | $6 |
| Hims | From $6 a use | $6 |
| PeterMD | $72.50 for 10, $190 for 30 | $6.33 to $7.25 |
Strut Health publishes every strength: 10 doses a month cost $30 at 5 mg, $60 at 10 mg and $120 at 20 mg, which it fills with four 5 mg tablets. Its prices include the physician consult and free shipping, and it doesn’t refund once medication leaves the pharmacy. TRT Nation sells 20 mg tablets 30 at a time with free shipping, but keeps a no-refund policy and doesn’t describe how its clinicians assess ED.
PeterMD shows its 10-tablet pack as a sale from $250 to $72.50. Even at the sale price, its tablets are the most expensive generic tadalafil we found. Hone Health lists tadalafil from $25 a month, on top of a membership: $155 a month for Premium, the tier Hone says gives full access to its medications (its ED page doesn’t say whether the $25 Basic tier covers ED medication), plus a $65 lab test to start.
Which costs less over a month
A few worked examples from the published prices:
- Four times a month at 10 mg: at Strut, you buy 10 doses for $60, so about $24 of medication goes on the four you take. At Rex MD’s $6 tablet price, four doses cost $24.
- Four times a month at 20 mg: TRT Nation’s bottle works out to about $13 for four tablets, though you pay $99.99 for 30.
- Every day at 5 mg: Hims from $40 a month, or 30 tablets at Rex MD’s $2 sale price for $60.
If you have sex several times a week, or want to stop planning around a pill, daily tadalafil can be worth the steady monthly cost. If it is a few times a month, as-needed doses usually cost less. A clinician can also tell you which suits your health; for example, the 5 mg daily dose treats BPH symptoms too.
Brand-name Cialis
Brand-name Cialis, made by Eli Lilly, cost from $29 a dose at Rex MD and $958 a month for daily use at Hims on October 18, 2026. Drugs@FDA lists Cialis as approved on November 21, 2003. Generic tadalafil is approved for the same uses at the same strengths. Our Cialis cost guide has more on brand-name prices.
Chewables and combination products
Compounded tadalafil chewables cost $4.47 a tablet at BlueChew and from $30 a month at Hims and $59 a month at MangoRx. Maximus sells compounded tadalafil from $53.99 a month, and Happy Head a daily capsule that combines tadalafil with finasteride for hair loss at $71.20 a month. These are compounded: a pharmacy prepares them, and they are not Cialis, not generics of it, and not FDA-approved.
We leave out products that combine tadalafil with another ED drug such as sildenafil or vardenafil. The Cialis label tells patients not to take it with other drugs of this type. See compounded ED chewables.
Side effects: daily vs as needed
The lower daily doses tend to cause fewer side effects than as-needed doses. In the Cialis label trials:
Scroll sideways to see the full table
| Side effect | Daily 2.5 mg | Daily 5 mg | As needed 10 mg | As needed 20 mg |
|---|---|---|---|---|
| Headache | 3% | 6% | 11% | 15% |
| Indigestion | 4% | 5% | 8% | 10% |
| Back pain | 3% | 3% | 5% | 6% |
Placebo rates were 5%, 2% and 1% for daily use, and 5%, 1% and 3% for as-needed use. Side effects matter for cost too: a dose you can’t tolerate is money wasted, so start where your clinician suggests and adjust.
Insurance
Federal law excludes drugs used only for erectile dysfunction from Medicare Part D, unless the drug is used for another FDA-approved condition. Tadalafil is approved for BPH symptoms, so a Part D or private plan may cover it when it is prescribed for that use. For ED alone, expect to pay cash. Most online providers don’t bill insurance.
Before you pay
- Decide with your clinician whether you will take it daily or as needed, then compare that price.
- Check the strength behind the price: Strut’s 20 mg dose costs four times its 5 mg dose.
- Look for consult, membership and shipping fees, and the billing period.
- Read the refund terms; several providers don’t refund once medication ships.
Tadalafil must never be taken with nitrates or riociguat, because the combination can drop blood pressure dangerously. The label also warns that drinking a lot of alcohol (5 units or more) with tadalafil can lower blood pressure.
The bottom line
As of October 2026, daily generic tadalafil started at $40 a month at Hims, and as-needed tablets cost $3 to $7.25 each, with Strut Health and TRT Nation the cheapest per tablet. Pick daily or as needed first, then compare the price at your strength and the terms behind it. Our ED rankings and ED hub compare the providers on care and terms as well as price.
