In short
- Testosterone injections cost $79 to $183 a month, medication and any required membership included, at the five online clinics that publish an injection price, as of October 2026.
- The lowest prices come with conditions: PeterMD’s $79 needs a 12-month plan billed upfront ($948), and Hone Health’s $28 injections come to $183 a month with the $155 Premium membership.
- All-in programs that include labs, visits and shipping cost $129 (Henry Meds) to $299 a month (the top of Rex MD’s $250 to $299 range).
- Plan on lab costs before you start: $65 at Hone Health, $99 at Rex MD and $129 at TRT Nation, while Henry Meds and MangoRx include labs in the monthly price.
- Testosterone is a Schedule III controlled substance, and online prescribing without an in-person exam relies on a DEA rule that runs through December 31, 2026.
Online testosterone replacement therapy (TRT) costs $79 to $183 a month for injections at the five clinics that publish an injection price, as of October 2026, with the medication and any required membership included. Programs that also include labs, visits and shipping in one price cost $129 to $299 a month. The headline number rarely tells the whole story, though: memberships, first-lab fees and 6- to 12-month plans billed upfront can move the real cost by hundreds of dollars a year.

What the monthly price usually covers
Most online clinics fold the clinician, the medication and shipping into one charge. MangoRx and Henry Meds go further and include lab work in the monthly price. Rex MD includes follow-up labs at 3 months and every 6 months after that, plus at least one video consult a year, but charges $99 (regularly $199) for the first labs and consult.
Others keep labs separate. Hone Health starts with a $65 biomarker test and physician video consult (listed at $165). TRT Nation charges $129 for TRT blood work or lets you upload your own recent results. Marek Health requires a lab panel purchase of at least $450 on top of its $299 start price.
Labs aren’t optional. The Depo-Testosterone label says to confirm low testosterone with morning blood tests on at least two separate days before starting, and to check hemoglobin and hematocrit periodically during long-term treatment. Budget for a first test and for repeat tests, whether they sit inside the monthly price or not.
What a first year costs
Monthly prices are easier to compare once you add them up. This is our arithmetic from the published prices, at the lowest plan each clinic offers, before any first-month promotions:
| Clinic | First-year cost | How we got there |
|---|---|---|
| PeterMD | $948 | 12-month plan billed upfront, plus its lab panel (price not on the TRT page) |
| Maximus | $1,199.88 | Two 6-month plans at $99.99 a month |
| TRT Nation | $1,328.88 | $129 labs, then 12 × $99.99 |
| Henry Meds | $1,548 | 12 × $129, labs included |
| MangoRx | $1,788 | 12 × $149, labs included |
| Hone Health | $2,261 | $65 test and consult, then 12 × ($28 + $155 Premium) |
| Fountain TRT | $2,388 | 12 × $199, plus lab costs it doesn’t state |
| Rex MD | $3,099 to $3,687 | $99 start, then 12 × $250 to $299 |
The cheapest year depends on whether you stay on treatment. A 12-month plan at $79 a month is the lowest total if you finish it, but PeterMD’s policy makes every sale final once medication arrives. If you stop after three months, TRT Nation ($129 labs plus 3 × $99.99, or $428.97, since its first purchase covers at least 2.5 months) costs less than any prepaid year.
Why prices for the same treatment vary
Memberships
Hone Health is the only clinic in our list that bills medication on top of a membership. Premium ($155 a month) covers regular lab testing, physician consults, retesting and follow-ups every 90 days, and full access to its medications, so injections cost $183 a month all in. Its Basic plan ($25 a month) covers lab testing every 6 months, members-only prices on select medications and the option to buy physician consults. Hone’s injection page lists testosterone under “Tiers 2-3” without saying whether Basic covers it, so we price it with Premium.
Plan length
PeterMD and Maximus price by commitment. PeterMD charges $79 a month on 12 months, $109 on 6 months and $139 month to month. Maximus charges $99.99 on 6 months, $149.99 on 3 months and $199.99 for a single month, and it offers 50% off the first month on 6-month plans for a limited time. Hims’ enclomiphene plan follows the same pattern: $99 a month on a 10-month plan paid upfront and in full.
Compounded vs FDA-approved
Most low-cost programs prescribe compounded testosterone. Hone Health, Maximus, MangoRx and TRT Nation say their products are compounded, which means a pharmacy prepares them for your prescription and they aren’t FDA-approved. Rex MD uses FDA-approved generic testosterone cypionate, testosterone gel 1.62% and Clomid, inside its $250 to $299 range. Henry Meds charges $179 a month for FDA-approved Kyzatrex capsules, against $129 for its compounded forms. Our testosterone injection guide covers the difference.
Dose
Maximus says its pricing varies by dose and plan length, and Strut Health prices enclomiphene by strength ($79, $99 and $119 a month for 6.25, 12.5 and 25 mg). Most other clinics publish a “from” price without saying whether it rises with dose, so ask what you will pay at the dose you are likely to need.
Enclomiphene and other options
Enclomiphene isn’t testosterone. It is a compounded pill that prompts your body to make more of its own, and it costs $79 to $197 a month at the providers that publish a monthly price, as of October 2026: Strut Health ($79), Hims ($99 on a 10-month plan), Maximus and TRT Nation ($99.99 each) and Hone Health ($42 plus the $155 Premium membership). PeterMD charges $278 for a 2-month plan, regularly $345. See enclomiphene prices and enclomiphene vs TRT.
Insurance, HSA and FSA
None of the clinics in this guide bills insurance for TRT. Rex MD’s pages note that its parent company, LifeMD, accepts insurance for qualifying patients. Maximus says its plans are HSA/FSA eligible, and Hone Health’s biomarker test is FSA/HSA eligible, while Hims says it doesn’t take FSA or HSA payments.
The legal fine print that affects cost
Testosterone is a Schedule III controlled substance. Online clinics can prescribe it without an in-person exam because of a DEA and HHS temporary rule (90 FR 61301) that runs through December 31, 2026. If it isn’t extended or replaced, an in-person evaluation will be required from January 1, 2027, which could add a visit to the cost of starting. A 12-month plan bought now runs past that date, so ask the clinic what happens to your prescription if the rule changes. Our telehealth rules guide explains it.
How to compare quotes
Before you pay, check these on each clinic’s site:
- The monthly price at the dose you are likely to reach, with any membership added.
- What the first labs cost, and whether follow-up labs are included.
- How long the plan runs, and whether it is billed upfront.
- The refund policy if you aren’t prescribed, and once medication ships.
- Whether the product is compounded or FDA-approved.
- Which states the clinic serves; several don’t cover all 50.
The bottom line
TRT online costs $79 to $183 a month for injections at the clinics that publish a price, and $129 to $299 for all-in programs that include labs and visits, as of October 2026. The lowest monthly figure needs a prepaid year, so compare the first-year total and the refund terms together. Start at our testosterone hub, and ask a licensed clinician which form of testosterone, if any, fits your labs and health history.
